Our Hospital Discharge Program is available to:
- NDIS participants who are ready to leave the hospital but require ongoing care.
- Individuals needing Medium Term Accommodation (MTA), Short Term Accommodation (STA) or Specialist Disability Accommodation (SDA) while awaiting permanent housing.
- Those requiring continuous support to manage daily activities and rehabilitation during recovery.
